2.1 CAF members may be required to serve in situations that can be dangerous to their life or health. In order to ensure that CAF members and former CAF members may be provided pensions and other benefits under the Canadian Forces Members and Veterans Re-establishment and Compensation Act, all injuries, disease or illnesses, as well as exposure or suspected exposure to toxic substances, must be reported and documented.
2.2 The commanding officer (CO) of a CAF member must ensure that a permanent record of the relevant circumstances pertaining to an injury, disease, illness, or exposure or suspected exposure to a toxic substance, is kept on the CAF member’s permanent personal file. The record must include all relevant information including the way in which the circumstances relate to the military mission or task.
3.1 Incidents of injuries, disease or illness as well as exposure or suspected exposure to toxic substances must be recorded on form CF 98, Report of Injury, disease or illness.
3.2 When a CAF member on temporary duty is injured, or exposed or suspected to have been exposed to a toxic substance, the CO of the host unit must ensure that form CF 98 is completed and distributed, and that an information copy is sent to the parent unit of the CAF member.
3.3 Any person making an amendment to information recorded on form CF 98 must sign and date the change.
3.4 The completed form CF 98 must be forwarded to higher authority within fourteen days of the incident and must not be delayed because the injured CAF member is unable to make a statement. The statement must be forwarded as soon as possible. Form CF 98 must be submitted even if the fourteen-day time limit is exceeded.
