Basic Health Questionnaire |
The basic health questionnaire is an integrated part of the cadet registration form used to identify any areas that may require medical or activity accommodation. Subsequent documentation may be required depending on the response(s) to this questionnaire. |

Detailed Health Questionnaire DND 2570 |
The detailed health questionnaire is used if responses to a cadet’s basic health questionnaire necessitates additional clarification. It is used to collect personal health information. This information allows Regional Medical Liaison Officers (RMLOs) to make informed decisions regarding the safety of cadets’ participation in program activities. A medical condition could potentially pose a risk to a cadet participating in certain activities/training at the Corps and Squadron as well as in courses offered at a Cadet Training Centres (CTCs). The form allows RMLOs to apply accommodation and/or limitations to a cadet. It reduces the risk of harm to the cadet, and liability of CJCR Gp. |
Retained until the cadet reaches the age of 25, unless utilized in a CF H Svcs Gp treatment record where it becomes part of the medical file held for 28 years after the cadet has reached the age of 18, or 28 years from the last documented decision if the cadet is over the age of 18. |

Anaphylaxis Consent Form |
The anaphylaxis consent form is used to provide the cadet and his/her parents/guardians with information regarding anaphylaxis. This information allows supervising staff to make informed decisions to allow the cadet to participate in activities/training where exposure to allergens can occur. |

Anaphylaxis Emergency Plan |
The anaphylaxis emergency plan is completed for cadets who have allergies that could produce an anaphylactic reaction. It is used to provide supervising staff information/instructions regarding their treatment/emergency plan. |

Food Sensitivity Form |
The purpose of this document is to provide the cadet and their parents/guardians with information regarding food sensitivities, intolerances and special dietary needs. This form will indicate that meal conditions may be required to be accommodated/acknowledged while participating in cadet training and activities. This information allows supervising staff to make informed decisions and accommodations regarding the cadets’ participation in activities/training where meals are provided. |

DND 4994 Cadet and Junior Canadian Ranger Medical Intake form |
The Cadet and JCR medical intake form is used to collect health information upon arrival at a training centre/session. The form identifies any emergency conditions or recent changes to a participant’s medical condition and identifies medication that they may require. The form is used to identify any areas that may require accommodation where medical conditions could potentially pose a risk to the cadet. |
Retained until the cadet/JCR reaches the age of 25, unless utilized in a CF H Svcs Gp treatment record where it becomes part of the medical file held for 28 years after the cadet/JCR has reached the age of 18, or 28 years from the last documented decision if the cadet is over the age of 18. |

Cadet Medical File |
A cadet medical file is produced when a cadet is treated or supervised by a CAF Health Services. The information contained in the file will vary and be related to the assessments and treatment received. This may include civilian hospital treatment records, staff notes, and the following forms: DND 4991 Cadet and Junior Canadian Ranger Treatment Record. |
Upon completion, cadet medical files are transferred to the RCSU. Files are held for 28 years after the cadet has reached the age of 18, or 28 years from the last documented decision if the cadet is over the age of 18. |

The Medical Supervision Record (MSR) is a stored record produced and prepared by medical staff, which enables the daily tracking and accounting of the supervised self-administration of Cadet medications. |

DND 4993 Cadet and Junior Canadian Ranger Medical Disposition Note |
A medical disposition is used while a Cadet or JCR is attending a training centre/session and documents specific medical recommendations and activity limitations as part of the treatment plan. |
Retained until the cadet reaches the age of 25. Unless it pertains to a cadet medical file as overseen by a clinician then held for 28 years after the cadet has reached the age of 18, or 28 years from the last documented decision if the cadet is over the age of 18. |
