What is a T4E?
A T4E slip is issued to everyone who received EI benefits or repaid an overpayment in the previous year, for example, from January 1st, 2016 to December 31st, 2016. This slip must be filed with your income tax return for that tax year. The T4E indicates the gross amount of benefits paid, the income tax deducted and the amount paid toward an overpayment (if applicable).
Through My Service Canada Account, you can view and print your T4E as early as February 1st, and submit a copy with your Canada Revenue Agency (CRA) tax return. You will need to login to or register for My Service Canada Account to access your T4E online.
Another option is to receive your T4E through the mail. All T4Es for taxation year 2016 will be issued by February 28, 2017. If you do not receive your T4E by this date, please call our telephone information service at 1 800 206-7218 from 8:30 am to 4:30 pm or go to your Service Canada Centre and we will issue a duplicate T4E.
What is an Itemized Statement of Benefits Paid?
An Itemized Statement of Benefits Paid is a breakdown of EI benefits received during a calendar year. You can get an Itemized Statement for an EI claim for up to the last 7 years.
When you receive your T4E, if you disagree with the information on it or would like additional information, you can view and print your Itemized Statement through My Service Canada Account. You will need to login to or register for My Service Canada Account to access your Itemized Statement online.
You can also request an Itemized Statement by calling our telephone information service at 1 800 206-7218 from 8:30 am to 4:30 pm or go to your Service Canada Centre and we will issue an Itemized Statement.
How to get a copy of a duplicate T4E and/or Itemized Statement
Through My Service Canada Account, you can view and print your T4E and/or Itemized Statement and submit copies with your Canada Revenue Agency (CRA) tax return.
You can also make your request by calling our telephone information service at 1 800 206-7218 from 8:30 am to 4:30 pm or in person to your Service Canada Centre. A representative will:
- verify your SIN, name, current address and access code;
- perform a detailed ID check, if your access code is unknown;
Representatives from the EI Call Center and local offices can issue a duplicate T4E and an Itemized Statement. Allow 4 weeks for processing and mailing from the date of your request for an Itemized Statement.
