Information for Law Enforcement Agencies

The Canadian Benefit for Parents of Young Victims of Crime is available to applicants who have suffered a loss of income from taking time away from work to cope with the death or disappearance of their child or children as a result of a probable Criminal Code offence.

To apply for the benefit, applicants are required to submit an Incident Report Form. The Law Enforcement Agency where the incident was reported must complete and stamp the form in on behalf of an applicant.

It is the applicant’s responsibility to submit the forms to Service Canada once they have been completed to the following address:
