2-Butoxyethanol Regulations

The following form must be used for the 2-Butoxyethanol Regulations
Schedule 2: Information to Be Contained in an Application for a Permit
This form is used to request a permit to manufacture or import a product set out in Schedule 1 of the 2-Butoxyethanol Regulations, in which the concentration of 2-butoxyethanol exceeds the limits set out in the same schedule. If at the time of coming into force of the Regulations, you are carrying out any of these activities, you must apply for a permit to continue the activity. If issued, the permit expires 24 months after the day it was issued and may only be extended once, upon application, for the same use of this product containing 2-butoxyethanol. The Minister may refuse or revoke a permit if there are grounds to believe that the applicant has provided false or misleading information.

The following forms apply to the Prohibition of Certain Toxic Substances Regulations, 2012
Schedule 3: Information Related to the Use of Certain Toxic Substances in a Laboratory for Analysis, in Scientific Research or as a Laboratory Analytical Standard
This form is used to declare the current or anticipated use of a toxic substance listed on Schedule 1 or 2 in a laboratory for analysis, in scientific research or as a laboratory analytical standard. This report must be submitted as per section 3 of the Regulations.
This form is being offered as an online service and is available in Portable Document Format (PDF) only, as a signature is required. Once you have accessed the form, you must print the form and fill in the fields by hand for submission by mail, or you may scan the completed and signed form and submit it by email.

Schedule 4: Information Required in an Application for a Permit or an Application for Renewal of a Permit
This form is used to request a permit to manufacture or import certain toxic substances. If at the time of coming into force of the regulations, you are carrying out any of these activities for certain toxic substances, you must apply for a permit to continue the activity. If issued, the permit expires 12 months after the day it was issued and may only be extended twice, upon application, for the same use of the toxic substance. The Minister must refuse a permit if there are reasonable grounds to believe that the applicant has provided false or misleading information, and if information required under the Prohibition of Certain Toxic Substances Regulations, 2012 has not been provided or is insufficient. For more detail, refer to sections 9, 10 and 11 of the Regulations.

Schedule 5: Information Relating to the Manufacture or Import of Toxic Substances or the Import of a Product Containing It
This form is used to report the manufacture or import of toxic substances or a product containing it, listed on Part 4 of Schedule 2, if they meet the reporting requirements identified in section 12 of the Prohibition of Certain Toxic Substances Regulations, 2012.
