New substances: notification forms for chemical and polymers
The New Substances Notification (NSN) Forms serve as an aid for complying with the New Substances Notification Regulations (Chemicals and Polymers) and for completing the NSN dossiers prior to their submission to Environment Canada.
These forms are being offered as an online service and are available in Portable Document Format (PDF). Once you have accessed the forms, complete them for submission by mail.

- Advisory note March 1996: company name change form
- The Domestic Substances List Reporting Form: to include a substance that was, between January 1, 1984 and December 31, 1986, manufactured in or imported into Canada, in a quantity of not less than 100 kg in any one calendar year, or in Canadian commerce or used for commercial manufacturing purposes in Canada
- Form C: Substances proposed for the Confidential Portion of the Non-domestic Substances List
