Privacy notice

The personal information you provide to Health Canada is:

  • handled in accordance with the Privacy Act
  • used by the Health Products Surveillance and Epidemiology Bureau under the authority of the Food and Drug Regulations

Why we collect your personal information

The Canada Vigilance Program will use your personal information to implement an email subscription list of hospital stakeholders, to distribute periodic newsletters, notifications concerning mandatory reporting and webinar invitations.

How else we use or share your personal information

The Canada Vigilance Program may share your personal information with other programs within Health Canada that support mandatory reporting by hospitals.

If you don't want to provide your personal information

If you choose not to provide your email address, we won't be able to send you updates.

Your rights

You have the right to:

  • access and request corrections and notations to your personal information
  • complain to the Privacy Commissioner of Canada if you feel your personal information has been handled improperly

About the newsletter

Health Canada’s Hospital Mandatory Reporting Newsletter is published quarterly by the Canada Vigilance Program. The newsletter shares information with hospitals about the requirements for reporting medical device incidents and serious adverse drug reactions to Health Canada.

If you wish to receive hospital-specific reporting statistics, as well as other mandatory reporting-related communications, please answer the questions below.

Mandatory reporting (MR) contact

Are you a MR contact for your hospital (required)

What is a MR contact? An MR contact is a hospital-designated individual in charge of:

  • receiving MR general awareness material
  • receiving facility specific reporting statistics
  • disseminating MR information within your hospital
  • participating (or delegating participation) in Health Canada webinars on MR by hospitals

If you represent multiple hospitals, please enter their name, and separate them using a semi-colon (;).

What is a Health Canada ID ? A Health Canada ID is an 8-character ID starting with the letters HC, for example HC12345678.

If you do not know your ID, please contact the Canada Vigilance Program (canadavigilance@hc-sc.gc.ca)

If you represent multiple hospitals, please enter your IDs, and separate them using a semi-colon (;).
