Step 6 – Get a Copy of your Documents to Support your Identity

Now, let's take a closer look at Step 6 in the Application Guide Getting a Copy of your Document to Support your Identity.

As part of your record suspension application, you must submit a clear photocopy of a document that supports your identity.

If you do not, your application will be returned to you as incomplete.

This identity document should be a valid government issued document (federal, provincial, or municipal), and it must have your name, date of birth and signature on it.

Examples of these include a driver's license or a health card.

To avoid delays in processing your application, make sure that the copy of your identification document is clear and that the information on it can easily be read.

If the board cannot read your document, your application will be returned to you.

Remember, do not send the original identification document with your application.

Send a photocopy only.
