Step 8 – Fill in the Record Suspension Application Form

Now, let's take a closer look at Step 8 in the Application Guide filling in the "Record Suspension Application Form". You should have the form in front of you as we go through this step.
You can find the form at the back of the Application Guide.
Make sure that you answer all questions and sign and date both sides of the form.
Print your answers using block letters in blue ink only.
If you run out of room on the form, you can attach additional pages to the form with the extra information on it.
Remember to make a photocopy of this form for your own files once you've filled it in.
Now, let's take a look at the Application form.
We'll start with Section A, "Personal Information".
Under number 1, "What is your full legal name?", write down your last name, followed by your first name in the spaces provided.
Under number 2 - "Have you ever used another name other than your legal name above?", tick either "yes" or "no". If you ticked "yes", write all of your previous names in the spaces provided.
Under number 3 - "What is your gender?", tick "male" or "female".
Under number 4 - "What is your date of birth?", fill in the year, month, and day that you were born in the spaces provided.
Under number 5 - "Were you born in Canada?", tick "yes" or "no". If you ticked "no", you must complete Step 5 in the Application Guide, entitled "Get Your Proof of Citizenship or Immigration Documents".
Under number 6 - "Do you have a driver's license?", tick "yes" or "no". If you tick "yes", write down your driver's license number in the space provided.
Under number 7 - "Are you Employed?", tick "yes" or "no". If you ticked "yes", write down the name of your employer in the space provided. Now let's take a look at Section B - Correspondence and Residence Information.
Under number 8 - "What is your mailing address?", write down the address of the place where you currently live.
Include the apartment number or house number, street address; city or town; province; postal code; and country.
All information about a decision will be sent to this, your current address.
Under number 9 - "Do you want information in English or French?", tick the language in which you want to receive correspondence from the board.
Under number 10 - "What addresses have you lived in the last ten years?", write down the addresses you've lived at over this period. For each one, include the apartment number or house number, street address; city or town; province; postal code; and country.
Include the date you started living there and the last date you lived there.
Start by writing down your current address, followed by all previous addresses.
Note that P.O. boxes are not acceptable.
Now, turn the form over to Page 2.
Under the "Applicant Information" section at the top of the page, write down your full legal name and date of birth again.
These must be the same as the ones that you wrote down on page 1 of this form.
Next, you will need to fill in the "Contact Information" section.
You must answer all questions in this section because the board will need to contact you directly.
Under number 11 - "Telephone number", write down the telephone number where the board can reach you, including the area code.
Tick "yes" or "no" to indicate if it is okay for the board to leave you a message at this number.
If you do not have a phone, write down your mailing address in the space provided.
Under number 12 - "Can we contact someone else about your application?", tick "yes" or "no".
If you ticked "yes", write down the name and telephone number of the contact person in the spaces provided.
Under number 13 - "Have you ever been a member of the Canadian Forces?", tick "yes" or "no". If you ticked "yes", you must also write down your Military Service ID Number, your date of enrolment, and your date of discharge.
You must also write down the complete mailing address of your unit.
If you ticked "yes" you will also need to complete Step 3 in the Application Guide, entitled "Get Your Military Conduct Sheet".
Next, you will need to fill in the "Conviction Information" section.
Under number 14 - "Do you have any other convictions that do not appear on your Criminal Record?", tick "yes" or "no". If you ticked "yes", you must write down the offence, arresting police, sentence, date, and court including the street, city and province the court is in.
You must do this for each conviction that does not appear on your Criminal Record.
Next, you will need to sign and date the "Applicant Authorization" section.
Under number 15 - read the text.
It explains under what authorization the information in this form is being collected, how it will be shared, and your rights under the Privacy Act.
You must sign and date the form in the space provided.
This indicates that you have read and understood the text, and certifies that the statements made by you in the application are true and complete.
