Step 9 – Complete the Measurable Benefit / Sustained Rehabilitation Form

Now, let's take a closer look at Step 9 in the Application Guide Completing the Measurable Benefit and Sustained Rehabilitation Form.

You should have the form in front of you as we go through this step. You can find the form at the back of the Application Guide.

Make sure that you answer all questions and sign and date both sides of this form. Print your answers using block letters in blue ink only.

If you run out of room on the form, you can attach additional pages to the form with the extra information on it.

Remember to make a photocopy of this form for your own files once you've filled it in.

Now, let's look at the form itself.

Under Section A at the top of the page, under "What is your full legal name?", write down your last name, followed by your first name in the spaces provided.

Next to "What is your date of birth?", fill in the year, month, and day that you were born in the spaces provided.

Sign the form in the space provided next to the date.

Next, under Section B, Part 1, clearly indicate how a record suspension would provide you with a measurable benefit and how it would sustain your rehabilitation into society as a law abiding citizen.

Note that it is not enough to simply state that it will provide you with a measurable benefit and sustain your rehabilitation into society.

You must provide details of what the measurable benefit is; for example: to gain entry to a college program, or access a specific job opportunity, and how this would sustain your rehabilitation into society; for example: by helping you earn a source of income.

Include as much detail as possible.

Next, under Part 2, describe, in detail, all of the positive changes you've made to improve your situation since your conviction.

You can include supporting documents here; for example: employment; educational or vocational training; character references; volunteer work; maintenance programs, etc.

Now, turn the form over to Page 2.

At the top of the page, where it says "Indicate the full legal name and date of birth of the applicant provided on the front of this form", write down your full legal name and date of birth again in the spaces provided.

These must be the same as the ones you wrote on page 1 of the form.

Next, under Part 3, provide information on the offence, or offences, that you were convicted of.

You must describe the circumstances of each offence and how, and why, each offence was committed.

Include as much detail as possible.

Next, under Part 4, for all sexual offences, include the age of the victim, and provide official documentation if available.
