RRSP contribution receipt summary

You have to fill out a RRSP contribution receipt summary if you prepare one or more RRSP contribution receipt slips. The amounts to report on the summary are the total of the amounts in the corresponding boxes of the supporting slips. The totals have to agree with the number of slips and the amounts you reported in the boxes of the slips. If there are errors or omissions, we may contact you for more information.

Account number

Filers must file their RRSP contribution receipt information return using RZ account format 123456789PR0001. An account number has 15 characters in 3 parts:

• the nine-digit business number (BN)
• a two-letter program identifier (PR)
• a four-digit reference number

Enter your 15-character PR account number. If you have not received your BN RZ account number, call 1-800-959-5525.

Report type code

This is a required field and one of the following values must be entered:

• Original = O
• Amendment = A

Tax year

This is a required field and data must be entered. For clarification of the filing periods, see Contribution year.

RRSP specimen plan number

Enter the registered retirement savings plan specimen plan number.

Total number of RRSP contribution receipt slips

This is a required field and data must be entered. Enter the total number of RRSP slips filed with this RRSP contribution summary.

Issuer name

Enter the complete issuer’s name. Two lines of 30 alphanumeric characters are provided for the issuer’s name. Enter the issuer’s name – line 1 and issuer’s name – line 2. A third line, issuer’s name – line 3, of 30 alphanumeric characters is provided for “care of” or “attention” use.

Issuer address

Enter the complete address of the issuer. Two lines of 30 alphanumeric characters are provided for the issuer’s address. Enter the issuer’s address (lines 1 and 2), city, province or territory code, country code, and postal code.

Contact name

This is a required field and data must be entered. Enter the contact’s first name followed by surname for this return. Omit titles such as Mr., Mrs., etc.

Contact telephone number

Enter the complete contact telephone number. Enter the contact’s area code, telephone number and extension number.
