Box 11 – School Type

The one-digit indicator associated to your corresponding school type.

Box 13 - Name of program or course

The name of the program or course you were enrolled in at the designated educational institution.

Box 14 - Student number

Your student number associated to the designated educational institution.

Student name

Your surname, first name, and initial.

Box 17 - Social insurance number (SIN)

Your social insurance number.

Student address

Your full mailing address, including the province, territory, or U.S. state, Canadian postal code or U.S. zip code, and country.

Box 19 - Start-date of the session (yy/mm)

The start date of the session, two-digit year and month, that you were enrolled in the designated educational institution.

Box 20 - End-date of the session (yy/mm)

The end date of the session, two-digit year and month, that you were enrolled in the designated educational institution.

Box 21 - Number of months part-time

The number of part-time months that you were enrolled in the designated educational institution.

Box 22 - Number of months full-time

The number of full-time months that you were enrolled in the designated educational institution.

Box 23 - Eligible tuition fees, part-time or full-time

The amount of eligible tuition fees for the part-time or full-time session.

Box 24 - Total number of months part-time

The total number of part-time months that you were enrolled in the designated educational institution.

Box 25 - Total number of months full-time

The total number of full-time months that you were enrolled in the designated educational institution.

Box 26 - Total eligible tuition fees, part-time and full-time

Report eligible tuition fees by completing Schedule 11. Enter the amount from line 10 of Schedule 11 on line 32300 of your return.
