Section B - Part 1 - Organization

Organization identification

Question 1: Legal name (Organization’s full name, as it appears on legal documents)

What is your organization’s legal name? The legal name is usually the name on your registration with the Canada Revenue Agency (CRA), and used on funding cheques or bank accounts.

Question 2: Operating name (if different from legal name)

What is the operating (or common) name of your organization (if it differs from the legal name)?

Questions 7: Organization primary address

What is the address of your organization? Give a complete address in a format recognized by Canada Post. Look at the Canada Post website for information.

What is the mailing address of your organization (if it differs from your organization’s address)?

If you select “No”, another section will appear for you to fill out your organizations mailing address.

In 500 words or less, describe your organization’s primary activities. Please explain how your organization caters to persons with disabilities.

If “Yes”, complete the fields for each amount owing. If you answer is “Yes”, your application will not be rejected for this reason. However, you must provide details as to the type of debt owed and department or agency to which the amount is owed.

Section B - Part 2 – Project proposal

Project proposal identification

Question 20: Project title

Give a short title that describes your project.

Questions 21: Planned project start date (YYYY-MM-DD)

When do you plan to start your project?

Please note that projects cannot start earlier than the date the agreement is signed with ESDC and your organization.

ESDC expects to communicate decisions between May 2022 and June 2022.

Question 22: Planned project end date (YYYY-MM-DD)

When do you plan to end your project?

Please note that all projects must be completed by September 30, 2024 at the latest.

Question 23: Amount requested from Employment and Social Development Canada

For this question, please provide the amount you are requesting from ESDC for your proposed project.

This amount should not exceed $100,000 (over 2 years) and will automatically populate the first line Amount requested from Employment and Social Development Canada in question 31.

ESDC will also consider funding up to $200,000 (over 2 years) for projects that:

  • have a national scope (delivered in more than one province or territory)
  • include 1 or more partnerships among diverse groups from within the disability community (for example, small to medium-sized disability organizations; organizations supporting different types of disabilities; and organizations supporting persons with disabilities with intersectional identities)
  • have the ability to provide accessible and inclusive services, and
  • provide information in both official languages

If you are applying for this maximum amount, please indicate in question 24 which how you meet these criteria.

Project proposal description

Question 24: Project summary

In no more than 500 words, briefly describe the need for the project including its goals, expected results, and the targeted group. You must also explain:

  • the need for your project in the disability community
  • how your application meets CFP criteria
  • if applying for funding between $100,001 and $200,000 – please describe how you meet the criteria to be eligible for this amount
  • the partnerships your organization has secured to support the project
  • how your project will meet the following expected results:
    • outputs:
      • host NAAW activities (to take place during NAAW 2023 and/or NAAW 2024)
      • meet with ESDC officials at least 2 times prior to each event NAAW event, including 6 months and 2 months before each NAAW event to discuss planned activities
      • prepare an interim and final report on project activities, including best practices and lessons learned, to disseminate amongst project partners and the Government of Canada, and
      • participate in ESDC-hosted NAAW webinars throughout the year to share your knowledge, best practices and lessons learned to the broader disability community
    • outcomes:
      • increase awareness of accessibility and inclusion in Canada
      • improve understanding and knowledge amongst Canadians about accessibility and inclusion issues
      • reduce stigma and attitudinal barriers towards persons with disabilities, and
      • contribute to sharing of best practices and lessons learned related to NAAW activities within the disability community
  • your plan for reaching underrepresented communities (for example, LGBTQ2+ persons/groups, Official Language Minority Communities, other underserved or vulnerable minority groups, persons with disabilities in rural and remote areas, Indigenous populations (First Nations, Métis, and Inuit) etc.)
  • your plan for the participation of disability organizations in the overall project design and the delivery of associated activities, and
  • any other relevant information related to the delivery and success of your project
Question 25: Program objective and priorities

In no more than 500 words, describe how the project meets the objective(s) and/or priority(ies) of the funding program. You must explain in detail:

  • what the project aims to accomplish and provide context in which progress can be monitored and success can be measured
  • objectives should be clear, achievable and measurable, and should identify who will benefit from the project
Question 26: Project activities and timelines - (provide the activities that will be taking place as part of this project)

Project activities are the steps you will take to meet your goals. They should be specific, measurable, realistic and relevant. The following can help guide you on how to fill out the Project Activities and Timelines table in the Application for Funding form:

  • activities should be listed in bullet point form and organized in chronological stages (for example. Months 1 to 3, Months 4 to 6, etc.)
  • starting each bullet with a verb should help keep activities clear and concise
  • if any partners are involved, specify which activities will be carried out by the partner(s) and which will be carried out by your organization
  • if any of the activities involve participants, include a brief description of how participants will be involved and for what specific purpose, and
  • if any of the activities involve research, include a brief outline of the methods, processes or strategies that will be used, created or established (for example marketing strategy, research or survey)

For example:

Months 1 to 3:

  • create planning network

Months 4 to 6:

  • identify speakers, facilitators and other logistical requirements
  • etc.

Months 7 to 9:

  • secure venue, audio visual and/or other required equipment
  • etc.

Question 27: Will any of the proposed project activities be delivered at your organization’s primary address

Question 28: Will the project or any of its activities involve or benefit people in English or French linguistic minority communities in Canada, in some way

Select “Yes” or “No” .

We commit to enhance the vitality of the English and French linguistic minorities in Canada. We support and assist their development and foster the full recognition and use of both English and French in Canadian society. Official Language Minority Communities (OLMCs) are generally defined by their geographic location (Francophones outside Quebec and Anglophones in Quebec) and by their shared collective identity. This collective identity is linked to the use of an official language in a minority setting.

If your proposed project involves OLMCs, you should answer “Yes” to this question. Complete the question in the form, by providing the following information:

  • clearly identify the linguistic profile of the beneficiaries targeted by the project
  • document any specific needs of the OLMCs and specify how the project will meet these needs
  • demonstrate that the applicant has the necessary human and financial resources to meet the specific needs of OLMCs

Answering “No” to this question will not invalidate your application.

Question 29: Is your project targeting vulnerable and underrepresented groups

Select “Yes” or “No”

If “Yes”, select the specific target group(s) that applies to your project in your application.

Section B - Part 3 - Budget

Question 30: Project cost - (expenses) for the eligible activities or services of the proposed project

List each activity or service and its associated cost.

Leveraging (project cost sharing) is not necessary under this call for proposals.

Costs must be reasonable and support project activities.

Eligible costs

Eligible costs are those considered necessary to support the purpose of the project. For this CFP, eligible costs could include the following:

  • wages and benefits
  • professional fees
  • travel and accommodations
  • materials and supplies
  • printing and communication
  • equipment rental/lease/maintenance
  • administration costs
  • capital assets, and
  • disability supports for staff of the recipient
Ineligible costs
  • Project costs incurred before an agreement is approved and signed by ESDC
  • Costs associated with ongoing general operating costs/service delivery (for example, regular telephone and heating costs, rent, utilities, property taxes, insurance, equipment for staff, audit costs, etc.)
  • Costs incurred in preparing the proposal
  • Construction/renovation costs
  • Costs associated with fundraising activities/lobbying
  • Staff salary bonuses, and
  • Purchase of alcoholic beverages

Question 31: Funding sources (revenues) for the proposed project costs noted above

The amount of funding requested from ESDC in Question 23 will automatically be populated in this question. If there is additional funding leveraged from other sources, please provide the information here. Please note that leveraging funding from other sources for this CFP is not mandatory. If there are no other funding sources, please move on to the next questions.

Question 32: Use this field to provide any further budget details that you may find necessary in describing your project

This field is optional if you want to provide more information related to the budget.

Section B - Part 4 – Attestation

In order for your application to be eligible, an official representative who has the capacity and the authority to submit project proposals and enter into contracts and agreements on behalf of the organization must complete this section of the form. By doing so, they are attesting to the following 3 points:

  • I have the capacity and the authority to submit this Application for Funding on behalf of the applicant organization
  • I certify and warrant on behalf of the organization and in my personal capacity that the information provided in this Application for Funding and any supporting documentation is true, accurate, and complete
  • I have read the Applicant Guide and understand the program’s requirements

To be considered eligible, an official representative who has the capacity and the authority to submit proposals on behalf of your organization must confirm that:

  • they have the authority to submit the proposal
  • certify that the information provided is true
  • they have read and understood the program’s requirements

To do this, the official representative must:

  • agree with the attestation by providing their name, title and the date

As this is an attestation, no signature is required.
