General Information

Background

In September 2004, the Public Health Agency of Canada (PHAC or the Agency) was established and confirmed as a legal entity in December 2006, by the Public Health Agency of Canada Act. The Agency's primary goal is to strengthen Canada's capacity to protect and improve the health of Canadians and to help reduce pressures on the health care system. The Agency's activities focus on prevention and control of chronic and infectious diseases, injury prevention and public health emergency preparedness and response.

The Agency is one of six departments and agencies that make up the federal government's Health Portfolio and reports to Parliament through the Minister of Health. The Agency is managed by the Chief Public Health Officer of Canada and the President of the Public Health Agency of Canada.

Responsibilities

Public health involves the organized efforts of society to keep people healthy and to prevent injury, illness and premature death. It includes programs, services and policies that protect and promote the health of all Canadians. In Canada, public health is a responsibility that is shared by the three levels of government, the private sector, non-government organizations, health professionals and the public.

The Agency has the responsibility to:

• Contribute to the prevention of disease and injury, and to the promotion of health;
• Enhance surveillance information and expand the knowledge of disease and injury and Canada;
• Provide federal leadership and accountability in managing public health emergencies;
• Strengthen intergovernmental collaboration on public health and facilitate national approaches to public health policy and planning; and
• Serve as a central point for sharing public health expertise across Canada and with international partners, and to use this knowledge to inform and support Canada's public health priorities.

Institutional Functions, Programs and Activities

Health Promotion and Chronic Disease Prevention

Promote the health and well-being of Canadians of all ages by conducting surveillance and public health research and supporting community-based projects which address the root causes of health inequalities and the common risk and protective factors that are important to promoting better health and preventing chronic disease.

Chronic Disease Prevention

This Program seeks new solutions for the complex challenge of preventing chronic diseases in Canada. It works across sectors to leverage investments and expertise to develop integrated, upstream approaches that help reduce common risk factors of chronic disease such as physical inactivity, an unhealthy diet, smoking and obesity. The Program funds the development and testing of innovative ways to increase physical activity, healthy eating and smoke-free living through sustained behavior standard change and the creation of supportive social and physical environments that positively impact the health of Canadians. Project findings (both successes and smart failures) are shared to help build the evidence base of what does and does not work for whom, and in what contexts. Ultimately, the Program contributes to increasing healthier behaviors and preventing chronic diseases among Canadians through multi-sectoral partners adopting, scaling up, and sustaining effective healthy living policies and interventions.

Demonstration Projects for Chronic Disease Prevention

Description: Includes records related to monitoring, researching and providing accurate information pertaining to collaborative projects with Provinces/Territories, innovative programs and policies occurring across the country in the area of chronic disease prevention which are rigorously assessed and their impacts and results are shared nationally. Records also include information related to this program, which develops a set of common tools, methodologies, and approaches for facilitating evidence-based practice, common evaluations, and knowledge exchange to maximize the impact and uptake of these innovations nationally. Evidence for Health Promotion, and Chronic Disease and Injury Prevention

This Program collects and analyzes national surveillance data on maternal, child and youth health, seniors' health, oral health, chronic disease, injury, health inequalities, and associated social, behavioral, and environmental determinants of health. It also carries out research to build a strong evidence base for action on public health. The Program provides ongoing leadership, engagement and expert advice by collaborating with national and international partners, and contributing to the development and sharing of evidence-based knowledge products, guidelines, declarations, joint statements, and frameworks. By addressing emerging public health issues, priorities, and knowledge gaps, the Program aims to better inform the development of policy and program interventions within the Public Health Agency of Canada and across sectors to improve the health and health behaviors of Canadians.

Fetal Alcohol Spectrum Disorder (FASD) Initiative

Description: Includes records related to policy, coordination and collaboration in the development of the evidence based tools and resources to further support the capacity of communities, non-governmental organizations, and including research institutes and communities of practice to address FASD. Cervical Cancer Screening

Data Coordination and Access Program

Description: The Data Coordination and Access Program (DCAP) acquires and manages centralized data holdings which are used by analysts across the Public Health Agency of Canada (PHAC) as well as Health Canada via the Data Extraction and Analysis application (DEXA), the Statistical Analysis System (SAS), and Enterprise Grid (EG). Includes records related to monitoring, researching and providing accurate information. Records may also include information related to the development of data sources and the standardized, protected acquisition of data related to health, as DCAP operates as a data broker on behalf of PHAC. Health Promotion

This Program aims to reduce health inequalities and improve the physical and mental health of Canadians by funding community-based programs and population health interventions. The focus is on improving knowledge, skills and health behaviors at individual and population levels, addressing risk factors that lead to negative health outcomes, and building protective factors to improve and maintain physical and mental health. The Program supports Canadians of all ages, particularly in the areas of mental health, maternal and child health, Indigenous early child development, and healthy aging, with emphasis on specific populations who are more vulnerable.

Health Promotion

Description: Includes records related to monitoring, researching and providing accurate information pertaining to Canada's population health approach to health promotion. Records may also include information related to health promotion, which includes five key strategies: building healthy public policy; creating supportive environments; strengthening community action; developing personal skills; and reorienting health services. Early Child Development

Middle Childhood and Adolescence

Child Rights

Mental Health Promotion Innovation Fund

Health Security

Prepare for and respond to public health events and emergencies; address health and safety risks associated with the use of pathogens and toxins; and address travel related public health risks.

Biosecurity

This Program regulates the use of human and terrestrial animal pathogens and toxins to protect the health and safety of the public. Pathogens and toxins pose a risk to the public because of their ability to cause disease or death. The spread of infectious disease in Canada could result in substantial social and economic disruption and international repercussions. Pathogen and toxic agents are used in a wide range of Canadian sectors for many different purposes, including: teaching and research at universities; disease diagnosis at hospitals and public health facilities; and vaccine development in the pharmaceutical industry. Program components are designed to prevent the accidental release of these agents from a laboratory, including via an infected worker, and to prevent the deliberate release by way of an act of terrorism or other criminal activity. The Program also helps Canada meet its obligations under the International Health Regulations (IHR) and the Biological Toxins and Weapons Convention.

Border and Travel Health

This Program protects the travelling public by: informing travellers on how to protect themselves from travel-related public health risks; working with the conveyance industry to protect against risks associated with water, food and sanitation; and working with border partners to limit the spread of public health risks to meet IHR requirements. Emergency Preparedness and Response

This Program is responsible for strengthening Canada's capacity to prevent, mitigate, prepare for and respond to public health events/emergencies. Antimicrobial Resistance

Classes of Personal Information

Funding Recipient Performance Management Data

This class of personal information contains information about funding recipients in order to create knowledge development and exchange products (e.g. summary of results for each cycle, and dashboards with key data). The data also contribute to PHAC corporate reporting, such as the Departmental Results Report (DRR) and the Performance Information Profiles (PIPs) and also assists in various policy research initiatives, and for internal program performance measurement. The information is collected on a biennial basis.

Complaints

This class contains personal information relating to the administration of the Public Health Agency of Canada and may relate to the legislation in general, or to any category of product or service. Most filing is done according to standard industrial classification coding; the key reference for retrieval purposes is the subject-matter involved. There is limited cross-referencing to and recording of incoming correspondence that may contain personal information. (Such correspondence includes complaints and requests for information) Personal information held by the Agency may be provided to federal or provincial departments and agencies to ensure a proper level of service to the person identified.

Conference Guest Lists

The guest list is the listing of all the names of people who can be allowed into the venue. This class contains personal information relating to the administration of the Public Health Agency of Canada and may relate to conferences held by the Agency. Personal information held by the Agency may be provided to federal or provincial departments and agencies to ensure a proper level of service to the person identified.

Event Feedback Survey

The Event Feedback Survey is done to assess participants' satisfaction to conferences and others events held by the Public Health Agency of Canada. This class contains personal information relating to the administration of the Agency and may relate to conferences and to similar events held by the Agency. Personal information held by the Agency may be provided to federal or provincial departments and agencies to ensure a proper level of service to the person identified.

General and Routine Correspondence

This class of information contains correspondence addressed to the Minister, the Chief Public Health Officer, and/or to the Associate Deputy Minister from external organizations and individuals in the form of requests for information, opinions and other submissions related to a broad range of policy issues pertaining to the Public Health Agency of Canada's activities, wherein some unsolicited personal information has been provided by the correspondent. This form of information would be retrievable only if specifics of name, date of communication and subject were to be provided; it is not arranged or retrievable by personal identifiers. The retention periods of these classes of personal information are controlled by the record schedules of the general subject files in which they are stored.

Inquiries

This class contains inquiries concerning the Public Health Agency of Canada's activities. The names of individuals appear in the files. Personal information held by the Agency may be provided to federal or provincial departments and agencies to ensure a proper level of service to the person identified.

Lists of Consultants

The List of Consultants is the listing of all the names of people who can offer services to the Public Health Agency of Canada. This class contains personal information relating to the administration of the Agency and may relate to services to be rendered to the Agency.

Lists of Guests and Partners

The list of guests and partners is the listing of all the names of people who can be invited to events organized by the Public Health Agency of Canada. This class contains personal information relating to the administration of the Agency. Personal information held by the Agency may be provided to federal or provincial departments and agencies to ensure a proper level of service to the person identified.

Mailing Lists

This class of personal information contains the names and addresses of individuals who are on a mailing list to receive specific information or publications on departmental programs and activities. The lists are used by the Public Health Agency of Canada to distribute agency information and to respond to requests by individuals. The lists are updated on a continuing basis. The lists are maintained for a minimum of two years.

Public Opinions

This is a class of records generated by the Public Health Agency of Canada officials who conducted public opinion research on a vast spectrum of topics related to the mandate and mission of the Agency. Retrievable records consist of data-gathering instruments, such as, but not exclusive to, questionnaires that may contain personal information that reveals the identification of the research subject. Data contained in this class of records are not used for administrative purposes. Individuals seeking access to these records must provide the title of the research project in which they participated, the location and the date of the research, and any other data that may help to identify and locate the personal information they are seeking.

Public Requests (mailing lists to forward certain publications)

This class of personal information contains the names and addresses of individuals who are on a mailing list to receive specific information or publications on the Corporation's programs and activities. The lists are updated on a continuing basis and maintained for a minimum of two years.

Unsolicited Opinions

Some programs and activities result in the accumulation of personal information which is not contained in the institution-specific personal information banks described in the Info Source publication. This personal information is stored within subject files described as part of the Public Health Agency of Canada's Information Holdings where records are not normally retrieved by the name of the individual or other personal identifier. This form of personal information consists of enquiries concerning particular matters, as well as requests for publications and reports. The information is normally retrievable only if specifics are provided concerning the subject matter. Retention of this form of personal information is controlled by the records schedules of the subject files in which the information is stored.
