Work-Sharing enrolment sheet

To process Employment Insurance Work-Sharing benefits for each employee, employers must complete an enrolment sheet and submit it to Service Canada.

The enrolment sheet must be completed immediately following the signing and implementation of an approved Work-Sharing agreement. You will receive the enrolment sheet from the Program Officer at the time of signing the agreement. It contains the Social Insurance Number (SIN), name and normal weekly hours for each eligible employee taking part in the agreement and must match the final Attachment A approved by Service Canada. If the employees on your enrolment sheet differ from the final approved Attachment A contact the Regional Work-Sharing Unit in your area to make changes to the agreement. Submit the enrolment sheet through the Data Gateway or by courier.
