The Department of National Defence’s Airworthiness Investigative Authority has issued the report From the Investigator in relation to a fatal parachute training accident on March 8, 2017 near Yorkton, Saskatchewan which claimed the life of Master Corporal Alfred Barr, a Royal Canadian Air Force Search and Rescue Technician.

This document is a brief summary of the circumstances and factual information known at this time and does not provide information on what caused the accident, which is still under investigation.
