Flight Safety Final Report on November 2016 CF-18 crash

The Department of National Defence’s Airworthiness Investigative Authority has issued the final report in relation to the fatal CF-18 accident of November 28, 2016 near 4 Wing Cold Lake, Alberta.

This document presents the findings pertaining to the cause of the incident, as well as recommendations and measures to prevent similar incidents in the future.

Captain Thomas McQueen of Hamilton, Ontario was killed during training inside the Cold Lake Weapons Range.
