Subject: CASREP – (Classification of Casualty)

1. Place, Date, Time and nature of incident.
2. SN, Rank, Surname and full given names, DOB, religion, Military Occupation description and code, unit, and nature of casualty. (For Res F members include commencement date of current period of training or duty, and whether member is Class A, B, or C). Use separate para for each casualty.
3. Classification of illness, injury or death.
4. NOK - Full given names, surname, relationship, primary language, address and postal code of the primary NOK, and whether notified, same for secondary NOK.
5. Location of casualty or remains (if at hospital, undertaker, morgue, state name and address of institution)
6. Duty (yes or no – whether or not performing military duty at time of casualty, if yes state type of duty, or if leave state type of leave and dates)
7. Motor vehicle accident (if MVA state whether private automobile or DND casualty, and whether driver, passenger, or pedestrian.)
8. Insert name, location and telephone number of local POC.
