• Conditions for Participation: Your participation in the online questionnaire is voluntary. If you choose to participate, your participation is subject to your acceptance of and compliance with these Terms of Service and the Privacy Notice (collectively referred to as the Terms). When you participate in the online questionnaire you are deemed to have accepted the Terms
• Your Input: Your input, once completed, will be provided to Service Canada. Service Canada will use the data to inform the information design for a future Government of Canada website
• Access to Information: The answers provided and the data generated in the course of analyzing the results will be subject to the Access to Information Act

The information you provide is collected for the Government of Canada under the authority of Section 7 of the Financial Administration Act.

The information will be collected using Loop11, a third-party hosted online service for remote usability testing. Loop11 does not set or collect cookies for participants of Loop11 projects. You may refer to the Loop11 Privacy Policy for additional information.

The information you provide may be used and/or disclosed for policy analysis, research and/or evaluation purposes. However, these additional uses and/or disclosures of your personal information will never result in an administrative decision being made about you.

The nature of the Internet is such that Web servers automatically collect certain information about a visit to a website, including the visitor's Internet Protocol (IP) address. IP addresses are unique numbers assigned by Internet Service Providers (ISP) to all devices used to access the Internet. Web servers automatically log the IP addresses of visitors to their sites. The IP address, on its own, does not identify an individual. However, in certain circumstances, such as with the co-operation of an ISP for example, it could be used to identify an individual using the site. For this reason, the Government of Canada considers the IP address to be personal information, particularly when combined with other data automatically collected when a visitor requests a Web page such as the page or pages visited, date and time of the visit.

Personal information will be protected under the provisions of the Privacy Act and will be stored in Personal Information Bank PSU 938, Outreach Activities.

Your personal information is administered in accordance with Section 7 of the Financial Administration Act, link to site the Privacy Act and other applicable laws. You have the right to the protection of, correction of, and access to, your personal information, which is described in Personal Information Bank(s) PSU 938, Outreach Activities. Instructions for obtaining this information are outlined in the government publication entitled Info Source.

You have the right to file a complaint with the Privacy Commissioner of Canada regarding the institution’s handling of your personal information.
