TSB issues the final report (A02O0123) related to its investigation of an in-flight fire aboard an Air Canada 767

TSB A 05/2004 Ottawa - November 16, 2004 – The Transportation Safety Board (TSB), today released its Final Report of the investigation (A02O0123) into an in-flight cargo bay fire that occurred on May 13, 2002, aboard an Air Canada 767. The fire was detected by cockpit instruments and, within 50 seconds, the fire was extinguished using on-board fire suppression systems and crew emergency procedures. No injuries resulted from the incident. Early in the investigation, the TSB issued two recommendations to Transport Canada, the Federal Aviation Administration (FAA) and Boeing. Those recommendations dealt with the installation of heater ribbons for water systems in transport category aircraft and with the potential for debris and contaminated insulation debris to propagate fires in cargo holds. Safety action on cleaning and maintenance procedures has been initiated by the manufacturer, Transport Canada and the FAA. However, the TSB is concerned that the response from the FAA and Boeing is limited to Boeing 747 and 767 equipment with open cargo bay floors. The TSB would also like to see the FAA and Transport Canada review of the use of heater ribbons in all aircraft. The TSB is an independent agency that investigates marine, pipeline, railway and aviation transportation occurrences. Its sole aim is the advancement of transportation safety.
