Submit a nomination 

To nominate someone, please provide the following 4 items:

1. A completed nomination form (PDF – 1 MB)
2. A statement of nomination (maximum of 250 words) describing why the individual would be deserving of this award. Your statement should:
   • Highlight all the actions the candidate has taken to advance the goal of equality for women and girls in Canada.
   • Detail the candidate’s volunteer or paid work, the names of the organizations and/or communities involved and the responsibilities they had within these organizations.
3. Candidate’s résumé or curriculum vitæ
4. Letters of support (minimum of 2 letters and a maximum of 4 letters, 1 page each)
