CF-188 Hornet (CF188771) – From the Investigator

The occurrence flight was an air-to-ground training mission flown in the military airspace north of 3 Wing Bagotville.

The planned Close Air Support scenario intended to simulate an air-to-surface weapon using the Sniper Advanced Targetting Pod system. To accomplish this, a moving ground object was selected for the training scenario, which was not a designated laser target. During this, the combat laser non-eye-safe mode was inadvertently engaged for a short duration, instead of the planned eye-safe laser mode. The change in laser mode was undetected by the crew during flight and was only noticed after landing in the course of the mission debrief, and was immediately reported.

There was no damage, and there were no injuries reported.

The investigation is focusing on materiel & systems, and human factors.
