Defence Team Accessibility Feedback and Request Form
The Defence Team welcomes feedback on any accessibility barriers that you may have encountered in dealing with the Department of National Defence (DND) or Canadian Armed Forces (CAF) as well as feedback on the Defence Team’s accessibility plans and progress reports.
Type of feedback and requests you can submit via the webform
Feedback on accessibility plans, progress reports or any barriers experienced when interacting with DND and CAF services, programs, policies, practices, offices, Bases and Wings, websites, or events can be submitted via the webform.
Requests for documents/products in alternate formats or request for locating accessibility related resources can also be submitted via the webform.
How feedback will be used
Feedback will be used to improve accessibility at DND and the CAF. Feedback may be addressed right away or be used to advance future accessibility plans and improve overall accessibility for the Defence Team.
All feedback will be taken seriously and treated in a way that respects the privacy and confidentiality of individuals who submit feedback. Feedback received will be reflected in future accessibility plans and progress reports and will not be associated with an individual’s name.
Any personal information associated with the feedback will only be shared with Defence Team Accessibility Office employees directly involved in improving accessibility at DND and the CAF. If you wish to remain anonymous, you do not have to include your name or contact information when submitting your feedback.
Anonymous feedback
You have the option to submit your response via webform anonymously or with your contact information, based on your choice. If you wish to be contacted regarding your request or feedback, please provide us with your contact information in the designated space below.
If you wish to submit feedback anonymously the online webform is the best method to do so. That way we will not see any of your contact information unless provided to us. Your information will remain anonymous, as long as you do not include identifying details, such as your name.
Acknowledgement of feedback
Acknowledgement of receipt of feedback received via this webform will only be sent if contact information is provided. If you do not provide us with your contact information, you will not be contacted for a follow up.
Privacy and confidentiality
Information submitted via this webform will be received by the Defence Team Accessibility Office (DTAO). The information will be administered and protected in accordance with the requirements of the Privacy Act and Access to Information Act. Information provided will only be used for mandated activities relating to the implementation of the Accessible Canada Act.
To ensure your privacy and the privacy of others, please do not share any sensitive information (Protected A or B) in this form.
Examples of Protected A information include: addresses, age, date of birth, race, national or ethnic origin, religion, health card number, social insurance number, service number, cadet information number etc.
Examples of Protected B information include: medical information, information protected by solicitor-client or litigation privilege, and information received in confidence from other government departments and agencies.
Any sensitive information will be removed from the feedback upon receipt.
