Requesting Defence 365 Support From Your Personal Device (Without DWAN)

Defence Team personnel working remotely using Defence 365 (D365) and who do not have access to the DWAN can now receive technical support from EITSM/Assyst using the new EITSM D365 Non-DWAN Service Request Form (Accessible only on the National Defence network).
Submitting this form creates a ticket in EITSM/Assyst and a notification will be sent to your D365 email address with the ticket number. To receive status updates, simply call your local Service Desk (SMC Help Desk Contact Information (Accessible only on the National Defence network)) and provide the relevant ticket number.
To access the new form:
- From your Computer: click here (Accessible only on the National Defence network) to access the form.
- From a Tablet or Smartphone: please download the “Power Apps” Application on your device (Android/IOS (You are now leaving the Government of Canada website)). The new “EITSM D365 Non-DWAN Service Request Form” will be located under “My apps” within the “Power Apps” application.
