Message from the Surgeon General: Patient identification process

In order to continually improve the quality and safety of the health services we provide, it is essential that Canadian Forces Health Services (CFHS) staff ensure that the identity of a patient is confirmed and that their demographic information is up to date prior to their receiving care at one of our medical or dental clinics.

CFHS has developed and will soon be implementing a new standardized patient identification process intended to increase the efficiency of follow-ups as well as ensuring that the right care is provided to the right patient at the right time.

As of May 30th, when you visit a Canadian Armed Forces (CAF) medical or dental clinic, you will be asked to present your military ID. If you don’t have your military ID with you, another form of a government issued photo identification card, such as your driver’s licence, will also be accepted. Once your identity is confirmed, you will be asked to update all of your demographic information.

This process is a best practice that reflects the standards of the healthcare industry. It will also make it possible to adopt a consistent approach throughout all the Health Services Centres and clinics of the CAF.
