Download a copy of the application form to your computer:
Complete and submit the application form using a computer and email attachments. Please do not print it, fill it in and send it by other means.
Be sure to digitally sign each completed application and email it in PDF format to Health Canada at ard.ard@hc-sc.gc.ca. The application form must be digitally signed and dated by the appropriate representative of the organization who has the legal authority to sign.
Also submit any other required documentation with the application as e-mail attachments.

Incomplete applications may not be considered for further review.
Health Canada reserves the right to:
- reject any submission received
- accept any submission in whole or in part
- seek additional information for any project or applicant to inform the review
- cancel and/or re-issue this funding opportunities at any time
We are under no obligation to enter into a contribution agreement as a result of any invitation to submit an application.
Application deadline
The application form and required attachments must be submitted no later than 23:59 Pacific Time, on May 25, 2022, to be considered eligible.
Be sure that the application form is digitally signed and dated by the appropriate representative of the organization who has the legal authority to sign. Submit it electronically in PDF format to Health Canada by email at: ard.ard@hc-sc.gc.ca.
Health Canada will acknowledge receipt of the application within 2 business days.
Health Canada will not accept applications submitted after the official deadline.
Funding decisions will be communicated by email.

A prerecorded information session video is available upon request in English and French, with transcripts. The information session:
- provides additional background on the program
- outlines the 2022 call for proposals
- includes details on funding priorities
- gives tips on how to complete the application
