Introduction of the Downloadable IMM 5455 Supplementary Identification Form

Summary

The purpose of this Operational Bulletin (OB) is to introduce a new downloadable version of the Supplementary Identification Form. 

Background

The Supplementary Identification Form (IMM 5455) is used to collect client information related to the processing of Permanent Residence (PR) Cards. The printed version of this form has an integrated adhesive photo patch and a shaded box to assist with the digital capture of the applicant’s signature. Due to these special features, this form was not available in a downloadable format. PR Card applicants were required to contact Citizenship and Immigration Canada (CIC) to have the form sent by mail. This delayed the application process for applicants and resulted in increased postage and handling costs for CIC.

New Format

Effective February 1, 2012, the IMM 5455 will be available to applicants in a fillable and downloadable format. The shaded signature box is no longer a requirement, as the applicant’s signature feature will no longer appear on PR Cards issued as of February 1, 2012. Applicants will be asked to provide their photos in a small envelope to be sent along with their completed IMM 5455 form. Photos are not to be attached to the IMM 5455. Photos will now be mounted with a stick-on adhesive patch at Case Processing Centre (CPC)-PR Card in Sydney. The PR Card application and guide have been updated to reflect these changes.

Citizenship and Immigration Canada will therefore no longer be mailing the IMM 5455 as it is now downloadable. However, as stated in OB 352, the form will only be mailed in the following instances:

  • Individuals requesting Braille or large print format.
  • Individuals who self identify as all of the following:
    • do not have access to the internet or who do not have a printer;
    • do not have family members or friends who can assist them; and
    • unable to access application kits via an organization or business due to a mobility disability.
  • Individuals who reside in remote areas where internet access is unavailable. This can be verified on the Industry Canada website at: www.ic.gc.ca/app/sitt/bbmap/hm.html?lng=eng.

CPC-PR Card Sydney will maintain the capacity to process either version of the IMM 5455. Internal processing of photo corrections through Port of Entry and CIC offices may continue to use the hard copy version of the form.
