To stop the mailing of any of the items listed, select the Stop mailing this correspondence item radio button option.

To start the mailing of any of the items listed, select the Begin receiving this correspondence item by mail radio button option.

Daytime phone number including area code

Enter a daytime phone number, including area code. If you are outside Canada and the United States, also include the country and/or city code. Use only numbers.

Submit

If all the information is accurate, click Submit and you will receive a confirmation page.
