Please answer the following questions about your experience with the Service Referral Initiative. We want to hear from you to improve our services. Your participation is completely voluntary and you can choose to stop at any time.

This survey is anonymous. We do not collect any personally identifiable information (such as names, email addresses, or phone numbers) and your responses will not impact your access to government services. Your responses cannot be linked back to you.

The information you provide (your feedback) is collected under the authority of the Department of Employment and Social Development Act solely for the purpose of assessing and improving the service delivery of the Service Referral Initiative and this information will never result in an administrative decision being made about you.

Employment and Social Development Canada does not intend to collect any personal information from you through this questionnaire. In the text of your response, please do not include any personal information about yourself or anyone else. This includes a name, address, telephone number, email address, Social Insurance Number, or any other personal information that could identify you or another person. If personal information is accidentally provided, it will be deleted as soon as possible.
