Information sheet: applying for temporary import and export certificate
General information
This application is to be used for requesting a certificate to import and export musical instruments or items that are part of a Traveling exhibition when these instruments or items contain elephant ivory and/or rhino horn.
Note:
- Complete all applicable sections accurately and legibly. Incomplete application forms will not be accepted
- The application must be signed in order to be processed. Electronic signatures are accepted
- A copy of the Musical Instrument Certificate or Travelling Exhibition Certificate issued for these items must be submitted with this application
- For the certificate to be sent by courier, a courier company and account number must be provided. Note that only Fedex or Purolator can be used
- Service standard - Allow up to 15 days business days for the processing of this application

Part 1: Applicant information
- Fill in all the required fields in the Applicant Information, to ensure that the certificate will be issued
correctly and sent to the appropriate destination.
Part 2: Information on the foreign Musical Instrument or Traveling Exhibition certificate
- Indicate the number of the CITES Musical Instrument or Traveling Exhibition certificate which was issued to you in your country of residence
- Provide the Issuance date as indicated on the foreign CITES certificate
- Specify the Valid Until date as indicated on the foreign CITES certificate
- Indicate the country of issuance.
Part 3: Information on stay in Canada
- Provide a tentative date of arrival in Canada with the instruments or items
- Provide a tentative date of departure from Canada with the instruments or items
