The personal information you provide to the Tobacco Control Directorate & Tobacco and Vaping Compliance and Enforcement Program will be used by the program under the authority of section 4 of the Department of Health Act, the Tobacco and Vaping Products Act, and handled according to the Privacy Act.
Why are we collecting your personal information?
The information is being collected in order to contact you for the purpose of responding to your query and comments and may also be used for compliance and enforcement activities.
Will we use or share your personal information for any other reason?
In limited and specific situations, your personal information may be disclosed without your consent in accordance with subsection 8(2) of the Privacy Act.
What happens if you don't want to provide your personal information?
The provision of the personal information requested on this form is mandatory. The consequence for not providing the personal information is that the program will be unable to respond to your question or concern, and possibly be unable to follow up on a complaint and complete an investigation. Sensitive personal information, for example your Social Insurance Number, is not required and should not be included in your message.
What are your rights?
You have the right to access and request a correction and/or notation to your personal information. You also have a right to complain to the Privacy Commissioner of Canada if you feel your personal information has been handled improperly.
