Schedule 1 (IMM 0207)

This form must be completed and uploaded by:

  • you, the principal applicant

Section A – Applicant Information

  1. Enter your full family name (surname or last name) as shown on your passport, travel document or identity document.
  2. Enter all of your given name(s) (first, second or more) as shown on your passport, travel document or identity document. Do not use initials.

    Note: If you do not have a family name on your passport or travel document, enter all given names in the family name field and leave the given name field blank.

    If you do not have a given name on your passport or travel document, leave the given name field blank.

  3. Enter your passport/travel document number, if you do not know the number, leave this space blank.
  4. Enter date of birth. If you do not know the complete date of birth, use “01” to fill in the spaces for the unknown year, month or day. Include a letter of explanation saying why you could not include the complete date.
  5. Enter your country or territory of birth.
  6. Indicate your country of citizenship.
  7. Enter your current country of residence.
  8. Enter your telephone number, including the country code, area/regional code.
  9. Select your current marital status.
  10. If you have an email address, enter it here (for example, name@provider.net).

    Note: By indicating your email address, you are hereby authorizing IRCC to send you correspondence, including personal information, to this specific email address.

  11. Enter your current residential address (where you live) by typing the following information:

Section B – Requirements of the Public Policy

If you answered “Yes” to question 1, you must provide details of how and when you left Sudan.

If you answered “Yes” to question 2, you must provide the name of your anchor, describe the relationship with you and provide documentation to demonstrate the relationship between you and your anchor.

If you answered “Yes” to question 5, you must provide documentation to confirm your temporary resident status in Canada.

Section C – Durable Solution Assessment

  1. Please select the appropriate box. If you are a citizen of Sudan and physically present in Sudan at the time you complete your application please select “Yes” and go directly to question 5. If you are not a citizen of Sudan and/or not physically present in Sudan at the time you complete your application please select “No” and continue to question 2.
  2. Explain why it is possible or not possible for you to return to your home country.
  3. You must provide details on how many times you returned to your country of origin, the length of your stay(s), the reason of your return and any other information you believe important.
  4. Answer each question by checking the appropriate box. If you answered “Yes” to any question from 4a) to 4d), you must provide detailed information and the name of the country in the appropriate section.
  5. Answer each question by checking the appropriate box. If you answered “Yes” to any question from 5a) to 5c), you must provide detailed information and the name of the country in the appropriate section.

Section D – Immigration Loans

If you are deemed eligible under this public policy, you are eligible for a transportation loan under Canada’s Immigration Loans Program (ILP).

Note:

  • All travelers are responsible for their travel costs to Canada. The ILP Transportation Loan provides financial assistance to eligible applicants to cover the costs of transportation to Canada.
  • Travel is booked by the International Organization for Migration (IOM). There is a CAD $300 per person travel booking assistance fee paid to IOM that provides benefits and supports for travelers. This fee is added to the loan in addition to the airfare costs. For more details on what IOM’s travel booking assistance includes, follow this link: International Organization for Migration.
  • If you do not have a passport and will be traveling on a Single-Journey Travel Document issued by Canada, it is recommended that you use IOM’s travel booking assistance in order to access IOM’s support to apply for an exit permit.

The following options are available for your travel arrangements. Please confirm which option you will be using:

  • Option A: ILP Transportation Loan supported by IOM’s Travel Booking Assistance
  • Option B: Pay for travel on my own (no ILP) but with IOM’s Travel Booking Assistance only (available in limited locations)
    • If you choose to use IOM’s travel booking assistance, and are traveling from Ethiopia, Kenya, Nigeria, and Tanzania only, you have the option to pay for IOM’s travel booking assistance and your travel costs directly to IOM, rather than having these costs placed on an ILP transportation loan.
    • You will not have access to an ILP transportation loan.
  • Option C: I will make all of my own travel arrangements. I am not seeking any support from IOM.
    • You will not have access to an ILP transportation loan.
