Annex A: Logic Models

The Logic Model depicts the set of activities within the Healthy Living Program that are expected to lead to its intended outcomes. It identifies the linkages between the activities and the achievement of its outcomes, showing the chain of results connecting the activities to the final outcomes and identifying the steps that demonstrate progress towards their achievement.

The logic model has the following components:

  • Activities: The broad categories or groups of tasks carried out under Program. These answer the question “what do we do?”
  • Outputs: The products or services which are produced as a result of the activities.  These answer the question “what do we produce?”
  • Immediate Outcomes: These are the short-term outcomes that stem from the activities and outputs, and answer the question “why do we do this?”
  • Intermediate Outcomes: Also called strategic outcomes. The medium-term outcomes that are the next link in the chain of outcomes flowing from the activities and outputs, occurring after the immediate outcomes have been achieved.
  • Long-Term Outcomes: This is the end result that the Program is trying to achieve. They are outcomes that take a longer time to be realized and are subject to influences beyond the Program itself.
