In order to complete my online application I need to provide one or more of the following:
RC65 Marital Status Form
Request a paper version of the form by calling the CRA at 1‑800‑959‑2221.
RC66SCH Status in Canada/Statement of Income Form
Request a paper version of the form by calling the CRA at 1‑800‑959‑2221.
Confirm your Marital Status Document
Provide the following information for you and your spouse or common-law partner in a letter:
- Name;
- Social insurance number;
- Date of birth;
- Marital status and effective date;
- Mailing and home addresses; and
- Both your signatures.
Proof of Birth
Provide an acceptable proof of birth for the requested child(ren).
