Completion of this feedback form is voluntary. This form is used to collect feedback internally and externally on our TBS Accessibility Plan 2022–25, future progress reports, as well as accessibility and barriers to accessibility within the work carried out in each department. Please do not include any sensitive, confidential, or personal information about yourself or any other individual in your open text box responses. The information in this form is collected by the Treasury Board of Canada Secretariat (TBS) under the authority of the Financial Administration Act to collect feedback to comply with the Accessible Canada Act. Should individuals choose to share their email address with us, we will use it to acknowledge receipt of the individuals' feedback as well as communicate with the individual on any inquiries or questions relating to the individuals' feedback. We will also publish annual progress reports in the years between accessibility plans. In these progress reports, we will report on the feedback we receive and how we are taking it into consideration. If you do not respond to all the mandatory questions your feedback will not be received.

Any personal information collected in this form, if you have provided any, will be used and protected in accordance with the Privacy Act and as described in Personal Information Banks PSU 938 (Outreach Activities) and PSU 914 (Public Communications).

The collection of personal information is authorized by section 7(1)(a) and Section 70 of the Accessible Canada Act. This personal information is collected by the TBS Inclusion, Diversity, Equity, and Accessibility Secretariat to comply with the Accessible Canada Act by creating a mechanism to collect feedback and be able to respond back to the individual and acknowledge that their feedback has been collected and how it was handled.

Collection and use of this personal information is in accordance with the Privacy Act. The mechanism is used to collect feedback internally and externally on our Accessibility Plan, future progress reports, as well as accessibility and barriers to accessibility within the work carried out in each department.

The personal information collected is described in Personal Information Bank PSU 938 – Outreach Activities and will be retained for 7 years. Under the Privacy Act you have the right of access to, and correction of, your personal information. To exercise either of these rights, contact Treasury Board of Canada Secretariat’s ATIP Coordinator. If you are not satisfied with Canadian Heritage’s response to your privacy concern, you may wish to Contact the Office of the Privacy Commissioner of Canada
