Diversity self-identification

Purpose

The Treasury Board of Canada Secretariat (TBS), Office of the Chief Human Resources Officer is committed to an appointment process for pension and benefits boards and committees that is open, transparent and merit-based, that also will bring appointees of high quality who reflect Canada’s diversity.

The information collected is for statistical purposes and allows TBS to ascertain the diversity composition of its boards and committees.

Who should complete this form?

Members of the pension and benefits boards and committees and members of the Observer Program.

Instructions

To complete the form, fill out section A. Personal Information then check the box that applies to you in sections B through G. Please indicate in section H your consent and declaration and then either sign the form electronically or print the form and sign with a wet signature. Once completed, please send to: PBSAdvisory-SPASconsultation@tbs-sct.gc.ca.
