To process your claims quickly, please go to the ACOA Direct online portal for the submission of all claims. If you are not able to access ACOA Direct or the forms listed below, please contact us.

Pre-authorized debit/direct deposit authorization (PAD)

• This form allows ACOA to directly deposit payments into your account when processed. It also allows direct payment to ACOA. Please click here to access a  PDF version (PDF - 825KB) of this document.

Final Payment Certificate

• This form certifies that the work outlined in the Statement of Work has been completed and all suppliers fully paid. Please click here to access a PDF version (PDF - 186KB) of this document.

Declaration of overdue amounts owed to the Crown

Non-cash costs certification
