This clinic does not provide emergency care. For emergencies requiring immediate care, call 9-1-1 or visit your local emergency department.

Sick or injured members bought to the clinic during an emergency will be sent directly to the nearest hospital’s emergency services.

About the clinic

The 41 Canadian Forces Health Services Centre (41 CF H Svcs C) is one of thirty-two Canadian Armed Forces (CAF) clinics across Canada. We provide health services in the Montreal area to CAF members and other authorized individuals. The clinic is detachment of the Saint-Jean-sur-Richelieu clinic.

Ou mission is to deliver high-quality, patient-centered primary care focused on the health and well-being of the CAF community we serve.

How to book an appointment

Appointment requests are accepted by email only. Send your request to the appropriate email address listed below for each appointment type.

Important:

  • Include all requested information to avoid delays in processing your request.
  • Do not include confidential or Protected B information in your emails.

Missed or Late External Appointments – Medavie Blue Cross:

Members will be billed directly for any missed appointments. They are not covered for CAF members under the Federal Health Claims Processing Service.

Sick parade

Appointments can be booked for:

  • Semi-urgent, but not life-threatening health issues
  • Follow-up after a civilian hospital visit (ER, hospitalization, etc.)
  • Need for medical limitations following:
    • an external medical procedure
    • a civilian doctor consultation (Class A and Class B reservists – under 180 days)

Booking an appointment

  • Name (last name, first name)
  • Service number
  • Phone number
  • Reason for appointment
  • Whether you have infectious symptoms (answer yes or no only, do not include any health details). Examples of infectious symptoms include vomiting, diarrhea, flu, cold, cough, fever, symptoms after international travel).

You will receive a response within 48 business hours (usually the same day or the next business day).

Immunization and Community Health

Appointments can be booked for:

  • Routine immunization (vaccine updates)
  • Travel health
  • Vaccination campaigns (these are announced in advance through your chain of command)

Note: For immunizations related to deployment, book a DAG pre/post deployment appointment.

    • Destination

Departure Assistance Group (DAG) pre/post deployment

Appointments can be booked for:

  • Signatures on form 2050
  • Pre-deployment:
    • Medical
    • Immunization
    • Social Work
  • Post-deployment (up to 90 days after return from duty):
    • Urgent need to see a doctor for an issue or injury that occurred during deployment

Mental Health and Psychosocial

  • Sick parade for mental health and psychosocial
  • Follow-up appointments

Periodic Health Assessment (PHA)

Appointments can be booked for:

  • Category re-assessment
  • Temporary category follow-up
  • Removal of a temporary category
  • PME for aircrew
  • Occupational change (e.g., COMFOSCAN, VOT, etc.)

Optometry

If you have had an eye exam within the last two years, you will not be reimbursed for your appointment. You can contact Medavie Blue Cross to confirm the date of your last eye exam.
