New Transfer Form

Purpose

The purpose of this Information Bulletin is to announce the launch of the new RDSP Transfer Form and to provide Issuers with an overview of the form.

Background

Following consultation with RDSP Issuers, a new transfer form and annex that collects the required information for Issuers to complete the transfer of the assets held in one RDSP to another and allow the Issuer to administer those assets according to the governing legislation.

The information collected on the Transfer Form is to be used in conjunction with the information that Employment and Social Development Canada (ESDC) sends to Issuers in the Transfer Information Extract File upon receipt of the electronic transfer transactions.

New Form Number

The new Registered Disability Savings Plan (RDSP) Transfer Form and the Holder Consent to a Registered Disability Savings Plan (RDSP) Transfer Form, together replace HRSDC EMP 5499.

Use of the Form

The new RDSP Transfer Form is to be completed by both Issuers during the transfer of plan assets from one financial institution to another. The information exchanged using this form will allow the receiving Issuer to correctly submit the required transfer information on the new contract registration transaction, and to administer the RDSP on an ongoing basis.

The new Consent Form is to be used when one or more holders of the existing plan will not be named a holder of the new RDSP. Under these circumstances, the relinquishing Issuer will collect consent from all holders on the existing plan.

The new form is being provided in a fillable PDF format.

Accessing the form

The new form is available on the Service Canada website and will be available on the RDSP, Grant and Bond Issuers Page of the ESDC website.

Key Dates

The form is available for use immediately.

User Guide

The RDSP Provider User Guide and other training and support materials are being revised to reflect the new form. You will be notified when revised versions are available.
