Application forms: export of goldenseal and american ginseng

The following forms are to be used for the export of Goldenseal (Hydrastis canadensis) or American Ginseng (Panax quinquefolius) grown in Canada.

Export permit for Ontario-grown American Ginseng - used when the Ginseng to be exported was produced in Ontario by members of the Ontario Ginseng Growers Association

Export permit for Goldenseal or American Ginseng - used for Canadian-grown Goldenseal or American Ginseng that was produced in a provinces other than Ontario. This form should also be used when the Ginseng comes from multiple provinces, even if one of the provinces is Ontario.

Request for additional permit shipments - used when permits for multiple ginseng or goldenseal shipments have been issued and additional permits are required for additional shipments.
