Section 9: import of military equipment serviced with product(s) containing processed asbestos fibres while it was outside Canada
Use this form to provide information related to the import of military equipment that was serviced with a product containing processed asbestos fibres if:
- the product was used to service the military equipment while it was outside Canada for the purposes of a military operation, and
- there was no technically or economically feasible asbestos-free alternative available at that time in that place

Use this form to provide information related to the use of product(s) containing processed asbestos fibres to service military equipment if:
- there was no technically or economically feasible asbestos-free alternative available when the product was imported, sold or used

Use this form to provide information related to the use of product(s) containing processed asbestos fibres to service equipment of a nuclear facility if:
- there was no technically or economically feasible asbestos-free alternative available at the time of the import, sale or use

Use this form to provide information related to the display of processed asbestos fibres in a museum.
Section 13: use in a laboratory and/or import for use in a laboratory of processed asbestos fibres or product(s) containing asbestos
Use this form to provide information related to the use in a laboratory and/or import for use in a laboratory of:
- processed asbestos fibres
- a product containing processed asbestos fibres or
- a consumer product containing asbestos in greater than trace amounts
in scientific research or as an analytical standard.
Section 14: import or use of processed asbestos fibres by a chlor-alkali facility
Use this form to provide information on the import or use of processed asbestos fibres for use in diaphragms in a chlor-alkali facility that was in operation on December 30, 2018.
