Complete Parts A, B, C, and the Declaration and Signature section of the Request to Amend Valid Temporary Resident Documents, or Information Contained in the Confirmation of Permanent Residence or Protected Person Document [IMM 1436] application form, unless otherwise indicated. If your application is not complete it will be returned to you. The information below is provided to help you complete the form.

Part A – Personal details contained on your valid temporary resident document, Confirmation of Permanent Residence or protected person document

Question 1

Indicate your Client Identification number (client ID) or Unique Client Identifier (UCI), if known. Otherwise leave it blank.

Note: If you have not dealt with IRCC since 1973, you will not have a UCI or a Client ID.

Question 2

Indicate your surname(s) (family name) as it appears on the immigration document that you are seeking to amend.

Question 3

Indicate your given name(s) as it appears on the immigration document that you are seeking to amend.

Question 4

Indicate all other surnames (family names) you have ever used, including different spellings and explain what they are. For example: ‘Smith’ (birth name/maiden name), ‘Leroux’ (former married name).

Question 5

Indicate all other given name(s) you have ever used, including different spellings and nicknames.

Question 6

Check the box to indicate your gender/sex (F-Female, M-Male or X-Another gender) as it appears on your immigration document that you are seeking to amend. If your document does not display your gender/sex, indicate your gender/sex as it appears on your passport.

Question 7
Indicate your date of birth as it appears on your immigration document that you are seeking to amend, in the following format: YYYY-MM-DD (Year-Month-Day)
Question 8

Indicate your place of birth as it appears on your immigration document that you are seeking to amend. Include the city and jurisdiction (province/territory/state). If your document does not display your place of birth, indicate your place of birth as it appears on your passport.

Question 9

Indicate your country of birth as it appears on your immigration document that you are seeking to amend.

Question 10

Indicate your citizenship as it appears on your immigration document that you are seeking to amend. To be a citizen of a country means that you were either born in that country (in most cases) or you were granted citizenship in that country. If you have dual citizenship, select the country that issued the passport or travel document that you used to enter Canada/on your original application.

Question 11

Indicate the country that issued the passport/travel document you used upon entry to Canada/on your original application.

If you do not have a copy of this passport/travel document, indicate the country that issued your current passport/travel document.

Question 12

Indicate the passport/travel document number of the document you used upon entry to Canada/on your original application.

If you do not have a copy of this passport/travel document, indicate your current passport/travel document number.

Check the box to indicate if this passport/travel document number was the one used at the time of entry/on your original application or if it is your current passport/travel document number.

Question 13

Indicate the date of issue of the passport/travel document you used in box 12 in the following format: YYYY-MM-DD (Year-Month-Day).

Indicate the expiry date of the passport/travel document you used in box 12 in the following format: YYYY-MM-DD (Year-Month-Day).
Question 15

Indicate your marital status as it appears on your document to be amended. Not all documents show your marital status. If there is no marital status on your document, check the box that applies to your marital status when you entered Canada or applied for your original document:

  • Never married
  • Married: This means that you and your spouse had a ceremony that legally binds you to each other. Your marriage must be legally recognized in the country where it was performed, and in Canada.
  • Widowed: This means that your spouse has died and you have not re-married or entered into a new common-law relationship.
  • Separated: this means that you are married but officially no longer living with your spouse.
  • Divorced: This means that you are officially separated and legally ended your marriage.
  • Common-law partner: this means that you have lived with your partner in a common-law relationship for a minimum of one year.
Question 16

Indicate your current marital status, if it has changed since you entered Canada or since you applied for your original document.

Indicate the language of correspondence (English or French) you wish to use.

Question 18

Indicate your current mailing address, including:

  • Post Office (P.O.) Box number (if applicable)
  • Apartment or unit number
  • Street number
  • Street name
  • City or town
  • District (if applicable)
  • Jurisdiction (Province/Territory/State)
  • Postal code
  • Country

If the country in which you reside does not use any of these values, please ensure a complete address for the country in which you reside is included.

Note: If you are requesting an amendment to a valid temporary resident document, a Canadian mailing address must be provided.

Question 19

Check the box to indicate if your current residential address (where you live) is the same or different than your mailing address.

Indicate your current residential address (where you live) if it is different from your mailing address, including:

  • Apartment or unit number
  • Street number
  • Street name
  • City or town
  • District (if applicable)
  • Jurisdiction (Province/Territory/State)
  • Postal code
  • Country

If the country in which you live does not use any of these values, please ensure a complete address for the country in which you reside is included.

Check the box to indicate whether your telephone number is from Canada and the United States (US), or another country.

In the fields below, indicate your telephone number, including:

  • Type - Indicate if your telephone number belongs to your residence, cell or business.
  • Indicate the country code.
  • No. - Indicate your telephone number including the area code.
  • Ext. - Indicate the extension, if applicable
Question 21

If you have an alternative phone number, check the box to indicate whether your alternate telephone number is from Canada and the United States (US), or another country.

In the fields below, indicate your alternate telephone number, including:

  • Type - Indicate if your telephone number belongs to your residence, cell or business.
  • Indicate the country code.
  • No. - Indicate your alternate phone number including the area code.
  • Ext. - Indicate the extension, if applicable.

Clearly indicate your e-mail address using this format (if applicable): name@provider.net

Note: By indicating your e-mail address, you are hereby authorizing transmission of correspondence including file and personal information to be sent to you via e-mail to the address provided. If no email address is provided, IRCC will communicate with you by regular mail.

Ensure your e-mail address will be valid for the entire processing period.

Question 23

Indicate the date you originally entered Canada, as it appears on your immigration document that you are seeking to amend, in the following format: YYYY-MM-DD (Year-Month-Day). Not all documents show the date you originally entered. If there is no date of original entry on your document, enter the date you first entered Canada, if known.

Question 24

Indicate the date permanent residence or protected person status was granted, as it appears on your immigration document that you are seeking to amend, or, indicate the date your temporary residence document was issued. Enter in the following format: YYYY-MM-DD (Year-Month-Day).

Note: A protected person includes individuals found to be Convention refugees or persons in need of protection.

Check the appropriate box to indicate what type of document needs a correction.

Note: If you are requesting an amendment to a temporary resident document, the document must still be valid (not expired).

Provide an explanation of the reasons why you are requesting the amendment(s). You may attach a separate piece of paper if more space is required. Outline the error that was made, what information was provided in your application, and any appropriate details as to why a change is required.
