• Please report cases using normal local / provincial / territorial methods.

Reporting instructions for provincial / territorial public health authorities

• Reporting to the Public Health Agency is required within 24 hours of case notification to Provincial / Territorial Public Health.
• During regular business hours (8:00 – 5:00pm ET):
• fax completed form (without first page) to 1-613-952-4723 and send an email notification (do not attach form) to erp-pre@phac-aspc.gc.ca;

  OR

• contact erp-pre@phac-aspc.gc.ca to discuss alternative reporting arrangements.
• After regular business hours (8:00 – 5:00pm ET):
  • please contact the Health Portfolio Operations Centre by phone at 1-800-545-7661 or hpoc-cops@phac-aspc.gc.ca to report the event and discuss method for information transfer.
