List of names for integrity verification form
Suppliers must provide a list of names for verification when submitting a bid or offer for contracts or real property agreements subject to the policy.
Integrity declaration form
Suppliers must submit an Integrity declaration form when they are unable to certify compliance with the policy as part of their bid or offer.
Subcontractor integrity verification request form
Prime contractors must submit a Subcontractor integrity verification request form to verify the status of their first-tier subcontractors who are individuals prior to bid submission and before entering into a direct contractual relationship.
Subcontractor exception form
If a supplier who is a prime contractor requires the services of a subcontractor who is suspended or ineligible under the policy, the supplier must summit a Subcontractor exception form.
