If you have general questions about the Canada Disability Benefit and how it works, you do not need to provide identification (ID). However, if you have specific questions about your file, you need to show ID and answer verification questions.
Accepted forms of ID
Some forms of ID will be accepted on their own, while other forms of ID require validation using a secondary piece.
To be accepted, your ID must:
- be original
- be valid (not expired)
- be issued by a Canadian government (federal, provincial or territorial) or foreign government
- include your current legal name
- include a photo of you
- include your date of birth and/or signature
Examples of accepted ID where you only need to give 1 of the following:
- a Canadian or foreign passport (foreign passports not in French or English must be accompanied by a professionally translated version)
- a permanent resident card
- a certificate of Indian status issued by the Government of Canada
- a provincial or territorial ID card or drivers license
If you can't meet the ID requirements, you can give any 2 of the following accepted IDs or documents:
- a birth certificate (provincial or territorial)
- a certificate of Canadian citizenship
- a certificate of registration of birth abroad
- any other government-issued ID
