What you need to apply

The information you need to provide when applying to the Canada Disability Benefit depends on:

If you are applying for yourself

You can also ask someone you trust to help you with your application.

If you received a letter from Service Canada

The letter has a unique 6 digit application code that can be used to apply for the Canada Disability Benefit .

To apply, you'll need to provide your:

  • unique application code: found on the upper right-hand corner of the letter from Service Canada, inviting you to apply
  • Social Insurance Number (SIN): found on your SIN card, SIN confirmation letter, or on income tax documents
  • status in Canada: you'll need to provide your legal status in Canada

You may want to provide your:

  • direct deposit information: you'll need the branch number, institution number and account number. Service Canada asks everyone to sign up for direct deposit because it's the fastest and most reliable way to get payments

If you did not receive a letter from Service Canada

To apply, you'll need to provide your:

  • Social Insurance Number (SIN): found on your SIN card, SIN confirmation letter, or on income tax documents
  • mailing address
  • status in Canada: you'll need to provide your legal status in Canada

You may want to provide your:

  • net income (line 23600) from your 2024 notice of assessment. Providing this will speed up application processing and payments if eligible.
  • direct deposit information: you'll need the branch number, institution number and account number

If you are applying as a legal representative on someone's behalf

We'll refer to the person you (the legal representative) are applying on behalf of as "the applicant".

What is a legal representative

A legal representative, for the purposes of the Canada Disability Benefit, is an individual or organization appointed by a legal document.

The limit of the actual authority is defined in the legal document. Legal representatives have the authority to represent and make decisions on behalf of an applicant.

A legal representative could be appointed:

  • as the guardian, trustee or committee by a court
  • through a protection mandate
  • through a Continuing or Springing Power of Attorney

If the applicant received a letter from Service Canada

The letter has a unique 6 digit application code that can be used to apply for the Canada Disability Benefit.

To apply, you'll need to provide:

  • the applicant's unique application code: found on the upper right-hand corner of the letter from Service Canada, inviting the applicant to apply
  • the applicant's Social Insurance Number (SIN): found on the applicant's SIN card, SIN confirmation letter, or on income tax documents
  • status in Canada: you'll need to provide the applicant's legal status in Canada
  • a copy of your proof of legal authority to act on behalf of the applicant:
    • examples of legal documents:
      • the court order that appoints a person, agency or institution as a trustee, committee or guardian
      • a certificate of guardianship
      • a Continuing or Springing Power of Attorney or mandate document that meets the legal requirements under the provincial or territorial Act in which it's executed
      • a dated letter or notice from the public trustee or public curator that contains the required information
      • a confirmation of the tutorship
  • a copy of your (not the applicant's) government issued identification (ID): it must be non-expired and include your name and date of birth:
    • examples of accepted ID
      • provincial or territorial driver's license
      • provincial or territorial ID card
      • certificate of Indian status
      • Canadian passport
      • foreign passport (if it's not in French or English it must be accompanied by a professionally translated version)
      • certificate of Canadian citizenship
      • permanent resident card
      • provincial or territorial birth certificate
      • copy of an act of birth (Québec)
      • certificate of registration of birth abroad
      • foreign birth certificate
  • your mailing address

You may want to provide your or the applicant's direct deposit information. Service Canada asks everyone to sign up for direct deposit because it's the fastest and most reliable way to get payments.

If the applicant did not receive a letter from Service Canada

To apply, the following additional information will be needed:

  • the applicant's Social Insurance Number (SIN): found on their SIN card, SIN confirmation letter, or on income tax documents
  • status in Canada: you'll need to provide the applicant's legal status in Canada
  • a copy of your proof of legal authority to act on behalf of the applicant:
    • examples of legal documents :
      • the court order that appoints a person, agency or institution as a trustee, committee or guardian
      • a certificate of guardianship
      • a Continuing or Springing Power of Attorney or mandate document that meets the legal requirements under the provincial or territorial Act in which it's executed
      • a dated letter or notice from the public trustee or public curator that contains the required information

If you are acting in your professional capacity within an organization

In addition to the information listed in the section above which applies to your situation, you'll also need to provide:

  • your organization's mailing address (instead of your own mailing address)
  • a copy of a document identifying you as part of the organization:
    • examples of accepted documents:
      • your employment contract with the organization
      • correspondence from the organization that recognizes you as their representative
      • an invoice from the organization with your name

You may want to provide your organization's business number: for example, Canada Revenue Agency (CRA) business number, CRA tax number, or provincial or territorial business number.

  1. Complete a printable Canada Disability Benefit application form or the Canada Disability Benefit Legal representative application form
  2. Drop it off at a Service Canada office or mail it to us at:
