New Dedicated Customer Call Centre Number and Dependant Eligibility Verification Program, January 2015

Date:

To: Compensation Managers, Heads of Human Resources, Participating Employers

The purpose of this notice is to inform you that starting January 2015 Great-West Life (GWL) will be implementing a Dependant Eligibility Verification Program (DEVP). The program will confirm that a dependant, for whom a plan member has submitted a claim, meets the eligibility criteria of the Plan.

As an employer-sponsored plan covering over 600,000 members and dependants, ensuring that dental claims are paid on behalf of eligible plan participants is responsible plan management and contributes to the sustainability of the Plan for all participants.

The program will have GWL send letters to 100 plan members each month. This exercise will use randomly selected dependant claims paid in the previous month as the means of identifying the plan member to be audited. GWL will ask selected members to supply documents to support the eligibility of the dependant being audited. Contingent on the type of dependant (i.e., child, spouse, student, etc.) different types of supporting documents will be required. Failure to provide proof in the specified timeframe will result in suspension of future claim payments for the audited dependant until such time as the dependant's eligibility has been verified.

The privacy of personal information supplied to GWL will be protected at all times.
