3. Overview

General

3.1 DAOD 5031-9, Course Reporting and Certificates, provides the policy and direction on course reporting and certificates.

3.2 In general:

  1. A CAF member who completes a CAF course must receive a paper record of having earned a CAF qualification;
  2. when the performance of members on a CAF course is deemed by the TA/DTA to require a formal report for assignment of duties or career administration purposes, a Form CF 377, Course Report, must be completed (no locally produced equivalents will be officially recognized);
  3. a Form CF 289, Certificate of Military Achievement, or its locally produced equivalent, is issued in recognition of the CAF member’s accomplishment on the course and provides the member with a paper record of having earned a CAF qualification; and
  4. as a minimum, Form CF 289, or its locally produced equivalent, must be issued if a Form CF 377 is not completed. However, it may also be issued along with a Form CF 377, for any course, at the discretion of the IT&E establishment.

Operating Principles

3.3 This instruction provides the guidelines for preparation and distribution of Form CF 377 and Form CF 289 (or its locally produced equivalent).

4. Responsibilities

4.1 The following table identifies the responsibilities associated with course reporting and certificates in accordance with DAOD 5031-9.

  • providing direction in each applicable CCD as set out in DAOD 5031-9
IT&E establishment
  • preparing and distributing Form CF 377 and Form CF 289, or its equivalent, in accordance with Annex A to this instruction 
  • completing the ATC function in HRMS MITE for Course sessions, this action will ensure the MPRR is updated in HRMS
CM
  • sending the Form CF 377 to PERMIS
Unit CO
  • forwarding the Form CF 377 to the supporting URS
URS
  • filing the Form CF 377 on the member’s UPF and ensuring that the member’s electronic record (on PERMIS/HRMS) reflects all appropriate corrections and/or updates 
  • if HRMS has not been updated, contact the IT&E Establishment for recent training events or the nearest MITE TASM for older Qualifications and Courses.
  • please note, if a member has received a Course generated by the Civilian Training module, it can only be recorded on the member’s MPRR if the training is managed and updated through the Civilian Administer Training module, not through HRMS MITE, this process will be reviewed after Version 8.9 is implemented

CAF member 

  • providing a copy of a Form CF 289, or its locally produced equivalent, to the URS for the CAF member’s unit personal file 
  • verifying the accuracy and completeness of one’s MPRR on HRMS when a Form CF 377 was completed, as well as on a regular basis
