Employers' Annual Reports

Each year by March 1, employers must submit two annual reports which cover the previous calendar year to the Labour Program. These two annual reports are:

1. Workplace Committee Report

The Workplace Committee Report summarizes the activities of the health and safety committee over the previous calendar year. This report is to be submitted to the nearest office of the Labour Program within your region.

2. Employer's Annual Hazardous Occurrence Report

Under the occupational health and safety regulations of the Canada Labour Code, federally regulated employers are required to submit the Employer’s Annual Hazardous Occurrence Report (EAHOR), no later than March 1 for the preceding 12-month period ending December 31, even if there are no hazardous occurrences. The report records the total number of disabling injuries, deaths, minor injuries, occupational diseases and other hazardous occurrences that have occurred in federally regulated workplaces in the past year.

Failure to comply with the requirement to submit your report by March 1 may result in a general inspection of your workplace to determine if there are other areas of non-compliance with your obligations under the Canada Labour Code.

Complete your EAHOR in the following steps

Step 1. Obtain the EAHOR form

Employers who have an e-mail address on file will receive an electronic package in January of each year. The package includes a prepopulated EAHOR in excel document displaying the active workplaces which were reported in the last submission.

Employers without an e-mail address on file will receive a reporting package by mail to their mailing address on file. The package includes a prepopulated EAHOR in paper displaying the active workplaces which were reported in the last submission.

Step 2. Complete the EAHOR

  1. Organization legal name – Indicate the organization legal name
  2. Organization common name – Indicate the organization common name if it is different from the organization legal name
  3. Organization ID – This is an ID which has been assigned to the organization by the Labour Program. If you do not know your Organization ID, leave it blank.
  4. Business number – A nine-digit number generated by the Canada Revenue Agency (CRA) that provides businesses with their own unique identifier. The Business Number is the first nine digits of the CRA program account number.

    Example of CRA program account number: "123456789 RP 0001". The Business Number is "123456789".
  5. Main Contact – The EAHOR mail-out package from the Labour Program shall be sent to this contact by email if email address is provided, otherwise by mail.
  6. Mailing address – Indicate the mailing address.
  7. Workplace ID – This is an ID assigned to each workplace by the Labour Program. It is used for reference purposes to uniquely identify each workplace.
    • If you do not know the Workplace ID, leave it blank.
    • If you have any new workplaces, indicate “New” in the “Workplace ID” column.
  8. Workplace name – Usually it is your organization name or a workplace specific name
  9. Headquarters – Indicate "Y" if the workplace is a corporate headquarters or "N" if the workplace is a branch
  10. Workplace reference number – It is the internal reference number for some large organizations to identify each workplace. It is an optional field found in Excel document only.
  11. Address of Workplace – You must report for all federally regulated workplaces

    In the Excel document the address is separated into the following columns:
    • Address – Street address (which is recognized by Canada Post)
      Example: Unit 1, 12345 Someplace Street
    • City – Full name
    • Province – Full name
    • Country – Full name
    • Postal Code – in the format A1A1A1
  12. Injury and Employment Data – See Section 2 for the full definitions at Additional information and resources

    If none, indicate "0". Do not leave any space blank for the following: Date ceased – If you have indicated "N" in the section "In operation", enter the date of closure as MM-DD-YYYY. Leave this space blank if you have indicated "Y".
  13. Comments – It can include any of the followings:
    • The legal name is changed, the organization is amalgamated
    • Data of workplaces are grouped
    • Any other changes related to the organizational structure of your organization
  14. Attestation – Once all the information has been entered and verified, check the checkbox.

    Save your completed EAHOR on your computer and name it as:
    • Organization ID_EAHOR2018_Offboard(or Onboard)
    Example: 00290_EAHOR2018_Offboard, or 00290_EAHOR2018_Onboard

Note: Keep a copy of the completed EAHORs in your records for the next 10 years

Step 3. Submit the EAHOR
