Employers' annual reports

Each year by March 1, employers must submit to the Labour Program two annual reports which cover the previous calendar year. These two annual reports are:

Work place committee report

The Work place committee report summarizes the activities of the health and safety committee over the previous calendar year. This report is to be submitted to the nearest office of the Labour Program within your region.

Employer's Annual Hazardous Occurrence Report

Under the occupational health and safety regulations of the Canada Labour Code, federally regulated employers are required to submit the Employer’s Annual Hazardous Occurrence Report (EAHOR), no later than March 1 for the preceding 12-month period ending December 31, even if there are no hazardous occurrences to report. The report records the total number of disabling injuries, minor injuries, deaths, occupational diseases and other hazardous occurrences that have occurred in the workplace in the past year.

Employers who have been late submitting their report in recent years or who fail to submit their report by March 1 may be targeted for compliance action as well as a general occupational health and safety inspection of their workplace to determine if there is other possible non-compliance with the requirements of the Code.

How to obtain EAHOR

Employers who have an e-mail address on file will receive an electronic package in January of each year.

Employers without an e-mail address on file will receive a reporting package by mail to their mailing address which includes a pre-filled paper EAHOR displaying the workplace addresses of sites which were reported in the last submission.

Once the electronic template is completed, save it on your personal computer as FJIDID_EAHOR2016_offboard. For example if your Employer identification number (FJID ID), is 000290 and the electronic template that you've completed is the Employer's Annual Hazardous Occurrence Report (EAHOR) – Regular then your file should be saved as 000290_EAHOR2016_offboard.

Note: Aviation, Marine and Rail Transport Employers with onboard Activities:

  • Please follow the instructions above, downloading the onboard EAHOR Excel template and saving as FJIDID_EAHOR2016_onboard (Example 000290_EAHOR2016_onboard).

How to complete the EAHOR

  1. Ensure the company’s full legal name is correct on the pre-populated report or enter this information in the blank template.
  2. Ensure the company mailing address is correct on the pre-populated report or enter this information in the blank template.
  3. Enter your name, title, e-mail address and phone number.
  4. Employer Identification Number (FJID ID) - This is a six digit field which has been assigned to you by the Labour Program and is unique to your company.
    • To find your unique number on the pre-populated paper form, look under the Employer Identification Number column. i.e. Employer Identification number 9-001560-0-01234, your FJID ID 001560.
    • If you do not know your Employer Identification Number (FJID ID), contact the Labour Program or call 1-800-641-4049 (toll free number) so that we may provide it.
  5. Workplace number - This is a five digit number (0-9) assigned to each workplace by the Labour Program, this is used for reference purposes to uniquely identify the workplace (mandatory if you have four or more worksites).
    • On the pre-populated paper form, the last 5 digits of the Employer Identification Number is the Workplace Number. Example: 9-001560-0-01234.
    • If you have any new workplace, type in “NEW” in the "Workplace number" column. Make sure that the address information and all other columns are updated. Please indicate the opening date of the new site in the "Workplace Comment" column.
    • If you do not know your Workplace number, communicate with the Labour Program so that we may provide it.
  6. Workplace name – fill in the name of the workplace (usually your company name or a site specific name)
  7. Workplace type – Please indicate which workplace is the corporate headquarters (HQ) and which ones are branches (BR).
    • On the pre-populated paper form, the second single digit in Employer Identification Number: 0=HQ, 1=BR. Example: 9-001560-0-01234, is HQ.
  8. Your workplace reference number – Your own reference number for uniquely identifying each workplace.
    • Optional field found in Excel document only
  9. Workplace address – Ensure that the workplace address of the company is properly written on the pre-populated report or enter this information in the blank template.
    • In Excel document the address is separated into the following rows:
      1. Workplace address line 1 - Street address associated with the Workplace (which is recognized by Canada Post).
        Example: 12345 Someplace Street
      2. Workplace address line 2 - Street address associated with the Workplace (which is recognized by Canada Post).
        Example: Unit 1
      3. Workplace city
      4. Workplace province – in the format XX
      5. Workplace country – in the format CANADA
      6. Workplace postal code - in the format A1A1A1
  10. Enter the data for Number of disabling injuries; if none occurred, indicate “0”. Do not leave this space blank.
  11. Enter the data for Number of deaths, if none occurred, indicate “0”. Do not leave this space blank.
  12. Enter the data for Number of minor injuries, if none occurred, indicate “0”. Do not leave this space blank.
  13. Enter the data for Number of other hazardous occurrences, if none occurred, indicate “0”. Do not leave this space blank.
  14. Enter the data for Total number of employees, ensure this is expressed as "full-time equivalents" (FTEs), see Employer’s Annual Hazardous Occurrence Report: additional information and resources for definition and instructions. Do not leave this space blank.
  15. Enter the data for Number of office employees, ensure this is expressed as "full-time equivalents" (FTEs), see Employer’s Annual Hazardous Occurrence Report: additional information and resources for definition and instructions. Do not leave this space blank.
  16. Enter the data for Total number of hours worked, include all hours worked by all employees (part-time, casual, etc.) during that reporting year, see Employer’s Annual Hazardous Occurrence Report: additional information and resources for definition and instructions. Do not leave this space blank.
  17. Clearly indicate when a workplace has ceased operations listing the specific date at which this workplace has closed down. In Excel document proceed as described below:
    • In the section” In operation”, indicate “Y” if company is still in operation and “N” if it has closed.
    • If you have indicated “N” in the section “In operation”, enter the date of closure in the section “Date Ceased”. Leave this blank if you have indicated “Y” in the section” In operation”.
  18. The section “Workplace Comment” is mandatory if you have grouped worksites together, moved or changed an address or added a workplace.
  19. Once all the information has been entered and verified to the best of your knowledge validate the attestation.
  20. Ensure to keep a copy of the completed form in your records for the next 10 years.
  21. You are now ready to submit your report. The following section will help you with the electronic submission of your report.

How to electronically submit the EAHOR

  1. You are now ready to submit your file. Please click on the hyperlink in the Excel template which will direct you to the Data Gateway login page. To access the Data Gateway website you will need your unique User name and password which was mailed to you in a separate envelope, in January 2013. To obtain a user name and password please contact the Labour Program or call 1-800-641-4049 (toll free number).
  2. Upon accessing the Data Gateway for the first time you will be required to enter the username and password provided. You will immediately be asked to change your password. Once your password is changed you will be able to upload your file(s). You will be asked to review your contact information and update it accordingly.
  3. Once logged in you will be prompted to select from a drop-down menu. Select the company for which you want to submit a report.
  4. For account configured for multiple services: Select EAHOR from the drop-down menu.
  5. Select the number of files you wish to upload then click on ‘Continue’.
  6. On the next page enter "1" and click "Continue"; on the next page click "Browse" and select the location on your computer where you saved the completed Excel file then click "Upload Now". Employers with onboard activities, please upload both your Offboard and Onboard files. (The completion of the Onboard template is only applicable to employers of Rail, Marine and Air).
  7. You should verify that the Data Gateway message indicates the file was uploaded successfully.
  8. The Labour Program will be in receipt of your submission.
  9. You can log in at any time in the future to view your submission.
