Appendix A: Logic model

Appendix A is the logic model for the Centre for Communicable Diseases and Infection Control, which depicts the Centre’s mandate to create and share credible knowledge and facilitate coherent, national action which contributes to the prevention and control of specific communicable diseases, with a focus on key populations at risk. The logic model highlights the Centre’s target groups which include: key populations at risk of and living with communicable diseases associated with community and health care settings, provinces and territories, Canadian non-governmental organizations, professional organizations, academic institutions, international health organizations, other national governments and health professionals. The logic model identifies the Centre’s short, medium and long-term outcomes as follows: knowledge on state, incidence, trends of communicable diseases and specific public health communicable disease practice in Canada; community capacity; appropriation/application of communicable disease advice/guidance; community communicable diseases learning, development/intervention; national/international stakeholder support, collaboration and responsiveness; and prevention and control of communicable diseases in Canada. And, the following strategic orientations guide the Centre’s work: create ongoing learning opportunities to improve performance; improve quality and effectiveness of strategic policy advice and issues management; strengthen knowledge translation, transfer and communication and reinforce and expand surveillance capacity.

Appendix A: Organizational Chart

Reporting directly to the Director General for the Centre for Communicable Diseases and Infection Control, Howard Njoo, are the following Divisions: Surveillance and Epidemiology Division, Professional Guidelines and Public Health Practice Division, and Programs and Partnerships Division. Chris Archibald is responsible for the Surveillance and Epidemiology Division who provide the following:

  • National Surveillance
  • Analysis and Dissemination of Surveillance Results
  • Epidemiological Research
  • Technical Epidemiological Support.

The Professional Guidelines and Public Health Practice Division, headed by Tom Wong, is responsible for the following:

  • Guideline Development for Professionals
  • Public Health Intervention Research
  • Identification and Dissemination of Best Practices
  • Technical Knowledge Transfer and Exchange Support
  • Modeling and Risk Assessment.

The Programs and Partnerships Division, headed by Marsha Hay Snyder, is responsible for the following:

  • Transfer Payment Programs
  • Public Awareness
  • Program Policy Advice
  • Stakeholder Relations.

Also reporting directly to the Director General is Steven Sternthal, the Associate Director General responsible for Business Services in addition to the Strategic Issues and Integrated Management Division that includes:

  • Centre Governance and Issues Management
  • Strategic Issues Development
  • Planning, Performance Measurement and Reporting.
